Safety Issue - Sloan Flushmate II 501-B Pressure Assist Flushing Systems (The Series 503 Flushmate III was previously recalled June 2012, January 2014, and July 2016.)
Issue: Affected Flushmate II 501 Series units can burst at or near the vessel weld seam releasing stored pressure. This pressure can lift the tank lid and shatter the tank, posing impact or laceration hazards to consumers and property damage. This Flushmate system would have been sold as part of a complete toilet tank from any of the following manufacturers: American Standard – tank 4086.100, and 4086.025, Crane Plumbing – tank 3-615 and 3-616, Mansfield – tank 1, and Kohler – tank 4413. The following Flushmate replacement part numbers: M-101526-F, M-101526-FA and M-102540-F may have also been sold. The production time period is September 3, 1996 through December 7, 2013. About 1.4 million (in addition, about 17,300 were sold in Canada).
